Purpose

Location Name Change Request Form

The Location Name / Address Change is available at the Location Name Change Form web portal. 

Please ensure to only utilize this form if the current location name is inaccurate or the address is moving to another location for that same location.
 

PDM Request Form

The Partial Data Move Form is available at the Partial Data Move web portal. 

A Partial Data Move request is required when an ADM location, associated POS hardware, and GMA consumer accounts all need to be moved from one organization in ADM to another (either within the same company for logistical purposes, or to another company when a location changes ownership). 

If a location has no GMA consumer accounts, accounts do not need to be transferred, or the kiosk hardware will be moving to a different location entirely, a relocation request should be submitted instead.
